The Red Cross Family Disaster Plan is a good place to start. The Red Cross Disaster Supplies Kit while designed for only 3 days is somewhat minimal but is also a good start. Red Cross assumes that their shelters will be open within 3 days and their kit is designed to support you until their shelters are open. If you don't want to go to a shelter you should prepare a larger and more comprehensive kit.
Emergency communicators must have a Jump Kit containing all the radio and personal gear they are likely to need.
